Quick Takeaways

  • Supportive Living at Delmanor Wynford offers personalized help with daily tasks such as medication, bathing, dressing, and mobility, while residents keep as much independence as possible.
  • Personal Support Workers are on site around the clock, Registered Nurses are on site during the day and evening and on call overnight, and a doctor visits at least once a week.
  • Each care plan begins with a comprehensive assessment and is reviewed every six months, or sooner when needs change, with families invited into the conversation.
  • Supportive Living residents share the same dining, fitness, and activity programming as Independent Living residents.

At Delmanor Communities, Supportive Living uses a resident-centred approach that focuses on each senior’s individual needs rather than a one-size-fits-all model. The Delmanor team provides high-quality care while maintaining a warm and welcoming environment where residents can continue living independently, stay socially connected, and enjoy a high quality of life.

What Supportive Living Looks Like Day to Day at Delmanor Wynford

Supportive Living focuses on providing personalized care while helping residents maintain as much independence as possible. Support can look very different from one person to the next, depending on the level of care they need and their unique care plan. In general, a senior in Supportive Living can receive help with:

  • Medication administration
  • Bathing, dressing, or grooming
  • Mobility
  • Wellness monitoring
  • Housekeeping and laundry

Support never means stepping back from community life. Supportive Living and Independent Living residents at Delmanor Wynford are fully integrated and take part in the same dining, fitness, and activity programming.

Yes. Delmanor Wynford has a team of Personal Support Workers (PSWs) and Registered Nurses (RNs) to assist residents. PSWs work rotating eight-hour shifts: 7 a.m. to 3 p.m., 3 p.m. to 11 p.m., and 11 p.m. to 7 a.m. RNs are on site during the day and in the evening, and overnight an RN is on call to handle any emergencies.

A doctor also visits Delmanor Wynford at least once a week.

What Specialized Healthcare Services Are Available?

A variety of additional healthcare services are available to residents at Delmanor Wynford as needed. Each is billed separately. The services include:

  • Mobile eye clinic
  • Dental clinic
  • Mobility clinic for mobility devices
  • Oxygen
  • On-site LifeLabs visits

For appointments outside the community, residents who don’t drive can use Delmanor Wynford’s transportation service to nearby external appointments.

How Is Care Coordinated With Outside Providers?

Delmanor Wynford can integrate its care package with publicly funded care services through Ontario Health at Home. When a resident’s care needs are at the higher end of the spectrum, Delmanor can also arrange private care support from external care providers.

How Is a Resident’s Care Plan Built?

The first step, before a resident moves in, is a comprehensive assessment. The Health and Wellness team sends a physician statement to the prospective resident and their family for their doctor to complete.

Once the physician has completed the form, the team schedules an in-person meeting with the resident and their family, either at the community or in their home. The resident is asked about their medical history, diagnoses, current medications, any recent falls or hospitalizations, and who helps them with bathing, toileting, transferring, or dressing.

Following that, a medication administration assessment determines whether the resident can manage their own medication safely.

The care plan is reviewed every six months, and whenever there is a significant change in health, cognition, mobility, or personal preferences. When a change occurs, the Health and Wellness team contacts the resident’s family and encourages them to be part of the discussion.

What Should I Ask About Supportive Living When Touring Retirement Communities?

It is important to make sure a community is the right fit for you or your loved one, and that it can provide the level of care needed. Some important questions to ask:

  • What services are included, and which are available for an additional fee?
  • Are there any community resources available?
  • What is the staffing model?
  • How are health changes communicated to families?
  • How does the community promote independence while keeping residents safe?
